Appointments

Office visits are by appointment. If you would like to be seen by one of our physicians for an emergent matter, please call our appointment desk and your request will be handled as such. If you are unable to keep your appointment, please notify the office as soon as possible. While we try to keep waiting times to a minimum, emergencies may occasionally disrupt our schedule. Please understand that we take your wait time very seriously and will inform you if there is a significant delay in the doctor’s schedule so you will have the option of rescheduling or waiting until the doctor is available.

If you are visiting us for the first time, please bring the following items with you:

  • Completed Registration and Patient History forms available for download
  • Insurance Card and Photo ID
  • Referrals from your primary care physician if required
  • X-rays, MRI’s and any other studies or tests that you may have completed

If you do not have the capability to print the Registration or History form, please arrive at least 15 to 20 minutes prior to your appointment time, so they can be completed in our office.

After Hour Emergencies

Our physicians are available on a rotating basis 24 hours a day, 7 days a week, including holidays. Please call (610) 644-6900 and you will be directed to our answering service. They will take your information and a physician will return your call as soon as possible.

Prescription Refills

Please call the office during normal office hours to obtain prescription refills. This will allow our physicians to consult your medical record and make the best decision regarding your care. Refills are not ordinarily handled after hours, weekends or holidays.

Insurance and Billing

Financial Policy

We are committed to providing you the highest level of service from your first visit to our office. We understand that insurance matters can be complicated and change frequently, so we will gladly assist you in obtaining your insurance benefit.

The responsibility for payment of fees, including co-payments and deductibles, is the direct obligation of the patient, not his or her insurance carrier. Please make sure you are familiar with your insurance benefits. If your insurance plan requires a referral, the referral must be provided prior to seeing the physician. It is the patients’ responsibility to obtain referrals. If you do not provide the required referral, we reserve the right to cancel or reschedule your appointment.

According to insurance contracts, you are required to pay co-payments, co-insurance and deductibles at the time of your office visit. Please come prepared to make payment as required. We accept cash, checks, Visa, MasterCard, American Express and Discover cards.

Patients without health insurance coverage are required to pay for services as rendered.

We appreciate prompt payment in full for any outstanding balance for services rendered by our practice. An account balance for more than 30 days is considered delinquent. If unresolved after 60 days, your account may be turned over to our collection agency for assistance.

It is not possible to list every insurance company. If you do not see your insurance carrier listed above, please call our billing department at (610) 644-6902 to confirm our acceptance.

Workers Compensation Claims

If you are visiting our office for an injury that occurred at work, please provide the following information at the time of your first visit: the date of the injury, name of your employer and contact person with telephone number, name of the insurance carrier covering your claim and the claim number that corresponds to your claim and/or injury. If you do not provide the required information, we reserve the right to cancel or reschedule your appointment.

We kindly request you provide us with your personal health insurance information and proper referrals if needed. In the event that your claim is deemed not work related, you will be responsible for all fees for services.

Please note: We do not accept letters of protection from your attorney for legal cases.

Automobile Accident Claims

If you are visiting our office for an injury sustained in an automobile accident, please provide the following information at the time of your first visit: the date of the accident, name of the insurance carrier covering your claim, the contact person and telephone number of the person representing the insurance company and the claim number that corresponds to your accident. You will also be required to provide us with your personal health insurance information in the event that your auto claim limits become exhausted. If your personal health insurance requires referrals, you are responsible to obtain referrals for all of service rendered to you by our practice. If you do not obtain the needed referrals, and your auto claim becomes exhausted, you risk not being covered by your personal health insurance and will be responsible for the payment of the unpaid bills.

Medical Record and Form Requests

Copies of your medical records are available to you or an authorized representative for a nominal fee. A signed release form, available for download, is required with your request. Special document requests, such as letters and forms will be honored for a fee.
